1. Allow 6-8 weeks for delivery because each badge is custom made.
2. Put only one badge style for each order form. If anything differs (lettering,
   size, color, style number) use separate forms.
3. State Seal - be sure to fill in the name of your state on the line beside
   "STATE SEAL". Also, be sure to check whether your seal should be
   (1) Plain (2) Plain with ENAMEL RIM or (3) Full Color (Enamel is Standard)
4. Fill out form completely and carefully. Be sure ALL necessary blanks
   are filled in and boxes checked.
5. Payment must accompany order, on individual orders.
